Patient Record Requests: Important Information

At SharePro Health, we are committed to ensuring the security and privacy of your medical records. To provide the most efficient and secure service possible, all record requests must be submitted online.


Important Information About Record Requests

  • Requests Cannot Be Made by Phone or Email: For the protection of your personal information, we are unable to process record requests made via phone, email, chat, or through the "Contact Us" page on our website. All requests must go through the online portal.
  • Why We Require Online Requests: Verifying your identity is essential to protect your medical information and comply with privacy regulations such as HIPAA. The online portal ensures a secure and traceable process, reducing the risk of unauthorized access.

How to Request Your Records

  • Visit Your Provider’s Website: Navigate to the medical records request section of your provider’s website. There, you will find the link to our secure request portal.  THIS IS NOT THE SAME AS THE PATIENT PORTAL - WE DO NOT MANAGE THE FACILITY'S PATIENT PORTAL. 
  • Submit Your Request Online: Complete and submit the required form through the portal to initiate your request. IF THERE IS NO PLACE TO REQUEST RECORDS ON YOUR PROVIDER'S SITE PLEASE CONTACT US USING THE FORM BELOW AND WE WILL SEND A LINK.

Checking the Status of a Previous Request

If you’ve already submitted a request and need an update:

Email us at status@shareprohealth.com with the following details:

  • Your full name
  • Date of birth (DOB)
  • Name of the facility
  • Your Tracking Number *if available


Can't find your tracking number or download code?

Be sure to check your text and/or email, including your SPAM/JUNK folder for a message from our system TRIMSnet. Many people miss the email because they are looking for SharePro or their facility- instead, search fot TRIMSnet. If you misplace your download code you can request it again through the system and a new one will be sent. Make sure to use the NEW code. If you are still having trouble, send us an email with the information listed above and we can resend your download code.
